Housing is the foundation of exceptional health outcomes. Thankfully, members of our community have access to affordable housing solutions through Lifewell. In 2024, Terros Health entered into a consolidation agreement with Lifewell to provide comprehensive whole-person care and critical wraparound services to more people in the community. The consolidation enables Lifewell to offer affordable housing while collaborating with behavioral health services providers to promote healthy, independent living. This includes long-term housing dedicated to Arizona’s most vulnerable citizens. Lifewell operates 46 dedicated adult units at three housing projects throughout the Valley, plus 14 units for those with visual impairments.
Housing
The H2O Program is an Arizona Health Care Cost Containment (AHCCCS) initiative that provides pre-tenancy and tenancy support services to individuals experiencing homelessness or housing instability.
Services include housing navigation, health-related social needs (HRSN) screenings, care coordination and life skills training. Our goal is to support members in securing stable housing and improve their overall health and well-being.
