Terros Health’s approach to serving patients extends beyond our health and counseling centers. It’s also about helping patients get the financial assistance they need, verifying insurance in a timely manner and answering any questions that arise.

Terros Health is dedicated to providing you with a full range of quality healthcare services at an affordable price, regardless of your ability to pay. If you are uninsured or underinsured, you may qualify for programs and services with reduced fees based on income and family size.

Insurance Accepted

We accept most major insurance plans, as well as Medicaid (AHCCCS) and Medicare. Because the insurance plans we accept may change, we ask that you contact our Patient Access Center at 602-685-6000 to determine if your plan is qualified under our contract with that company.

Terros Health will not discriminate in the provision of services to an individual (i) because the individual is unable to pay; (ii) because payment for those services would be made under Medicare, Medicaid, or the Children’s Health Insurance Program (CHIP); or (iii) based upon the individual’s race, color, sex, national origin, disability, religion, age, or sexual orientation.

AHCCCS Eligibility

Arizona Health Care Cost Containment System, or AHCCCS, is Arizona’s Medicaid agency, which offers several healthcare programs to serve Arizona residents. Programs are based on meeting certain income and other requirements but in general, you are likely to qualify for AHCCCS if:

  • Your family includes children under age 19
  • You have been a resident of Arizona for 5 years or more
  • Your family income meets the Arizona State guidelines. Our Eligibility Specialists can tell you more or visit the AHCCCS website for specific information

For additional information of what documentation will be needed for your application, click to download an AHCCCS Application Checklist (PDF).

Terros Health Discount Program

Terros Health offers a Discount Program or Sliding Fee Schedule (SFS) for services based on specific qualifications, including family income, size, and situation. The Discount Program may also help with reduced fees for services your doctor may order but which are done by other providers including pharmaceuticals, laboratory, and radiology.

Good Faith Estimate

You have the right to receive a Good Faith Estimate explaining how much your medical and mental health care will cost.

Under the law:

  • If you don’t have insurance or don’t plan to use insurance, you are entitled to a Good Faith Estimate for the total expected costs of non-emergency items or services.
  • You can ask for a Good Faith Estimate in advance of scheduling any service.
  • If you receive a bill that is $400 or more above your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy of your Good Faith Estimate for your records.
